Forms

The Office of Graduate Studies maintains many of the required forms for M.S. and Ph.D. students.  Forms pertinent to international students are also available.

Below we have outlined a sample timeline applicable to most doctoral students.  However, this guideline is not a substitute for regular meetings with your advisor and Supervisory Committee.  It is the responsibility of the student to make sure all required forms are completed and deadlines are met as outlined in the Graduate Program Document.

Rotation Approval Form – This form is completed during your orientation.  Dates change yearly, so be sure you have downloaded the current year’s form.

Appointment of Supervisory Committee –This form is completed during the second semester in consultation with your newly selected thesis advisor.

Program of Studies –  This form is completed during your first committee meeting and outlines your projected coursework.  It is to be submitted by May of the first year in the program.

Graduate Student Annual Report – Graduate students are required to hold a Supervisory Committee Meeting by May each year.  This form is completed annually and is intended to inform the student as to their progress in the program.

Application for Admission to Candidacy – Once a student has passed his or her comprehensive exam (by the end of the fifth semester in the program), the student can apply for Doctoral Candidacy.

Application for Degree – Please check the Graduation Services website for the applicable deadline.  Typically, this form is due at the beginning of the semester that a student plans to graduate.

Application for Final Oral Examination – Before the form is submitted to Graduate Studies, the two designated readers must read the dissertation and sign the form.  The chair or co-chairs must also sign prior to submission of the form, which must be filed at least three weeks prior to the scheduled final oral exam (dissertation defense).
